Plot
Junior Soprano's dementia is advancing. He leaves his house one morning looking for his brother Giovanni, known as Johnny Boy, who has been dead for decades. Junior is confused and frightened, wandering through his neighborhood asking people whether they have seen Johnny. The episode follows Junior through familiar streets that no longer feel familiar to him, trying to locate something he cannot precisely name.
Bobby Baccalieri finds Junior and helps get him home. The situation makes clear that Junior can no longer be left alone. Tony is contacted and must assess how seriously impaired his uncle has become. The legal and criminal implications of Junior's cognitive decline are significant: a man who cannot reliably remember what year it is is a liability in multiple directions. Tony visits Junior and speaks to him carefully, taking stock of what remains.
Tony Blundetto is working at the massage parlor and seems to be stabilizing. His old crew connections surface occasionally and he handles them with a combination of diplomacy and redirected energy. But New York's interest in Blundetto is not going away. Phil Leotardo has specific grievances from years ago, and his patience with the current arrangement is limited. Tony Soprano is trying to shield his cousin from what is coming without yet knowing exactly what form the threat will take.
